Application Process
Step 1: Review MAOT Approved CE Provider Program Policy and Procedures
Step 2: Fill out and sign the Application Form, download as a PDF and submit it with your Google Folder.
Step 3. Fill out and sign the Disclosure/Attestation Form, download as a PDF and submit it with your Google Folder.
Step 4. Create a Google Folder with required documents per Item 7 on Policy and Procedures. Email the Google Folder to info@maot.org. Write "APP-Name of Organization-Course Title" in the subject line. (ex, APP- XYZ Inc- Ethics Course)
Step 5: Pay applicable non-refundable application fee at Affinipay
Please note: CE approval application will not be reviewed until the completed application form, required documentation & applicable fee have been received.

COURSE FEES:
One course-1-3.0 hours- $75.00
One course-3.25-7.0 hours- $125.00
One course-7.25-12.0 hours- $200.00
One course-12.25-24 hours-$300.00
One course-24.25-36 hours-$600.00
Hours are based on the total number of session hours offered, not the number of hours that can be earned in one day

Expedited approval fee: 25% additional charge
Appeal fee: 25% additional charge

MAOT Approved Courses will be sent the MAOT Logo to put on their course materials and certificates.
